Social Work Services

​The Social Work Services Department is a link between the patient, his or her family, and the rehabilitation treatment team.  They facilitate the patient’s and family’s participation in the treatment program.

The Program Manager attends weekly team meetings and provides updates regarding the patient’s progress. Family conferences with the patient’s family and the treatment team may be arranged as needed. 

The Program Manager also provides the family and patient with needed resources, which may include Temporary Disability Funds, Social Security, Medicaid programs, Home Care Services, Transitional living, or Vocational Services.  In addition, he or she is the liaison with the insurance company for all recommended treatment authorizations required during the rehabilitation stay.

The Program Manager is available Monday through Friday, to provide support for the family as they adjust to the challenges of the patient’s hospitalization.  The Social Work Services Department also facilitates the monthly New Jersey Brain Injury Support Group and Stroke Support Group for Mercer County area residents who are survivors, and their caregivers.
